1. Referral into Unity Housing project Coordinator 

Referral to be accepted from Local authorities or other relevant groups or bodies suitable to identify support needs of those being referred.

2. Triage process to identify potential housing options 

Internally, the Unity Housing Support team will cross reference the referral to available property stock and allocate a worker to process the application and tenants support requirements. 

3. Referencing process commence 

The allocated worker will complete the necessary reference checks for the tenant, these can include references from local authority /charity, previous landlord, current employer, support worker. 

4. Interview with applicant to determine suitability and qualify information provided 

Once a referral has been reviewed and initial contact and checks have been made, a member of the management team will then interview the individual referred to ensure their suitability and engagement with the provision being offered to them. Once the outcome of the interview has been considered and eligibility has been confirmed, we would then make an offer to the individual. 

Referral Eligibility 

We work with many groups of clients. Our remit is to aid those who have low – medium support needs and whose risk level is also low. In order to be eligible for our services any given referral must: 
Be made by a local authority, organisation or body, who are well suited to assess the support needs and risk for the client they refer 
Have a clearly identified low – medium support need 
Be of low risk level 
Be eligible to rent in the United Kingdom and have recourse to claim welfare benefits 
Be willing to work with and engage with the support provided by Unity Housing Support 
Be willing to engage with other responsible organisations involved in their care, i.e. drugs or alcohol services 
Be age 18 or above 
Have basic living skills and be identified as Semi-Independent

We need to obtain and share information about you from, and with a number of organisations or agencies to enable us to assist you effectively. 
 
In order to help you to access housing and support services, we need your consent to access information about you from other agencies such as; Housing Benefit, Your GP, etc. 
Information that you provide to Unity Housing Support will need to be shared with the services that we will use to assist you. 
In order to ensure your safety and the safety of others we will always complete a risk assessment which can and will be shared with any services that we will use to assist you. 
Information will only be shared on a need to know basis, where there is a specific and legitimate reason to disclose relevant information.
